Don’t have insurance? Don’t worry, we offer our patients an “In-House Discount Plan.” This membership discount plan is for either individuals or families. As a member, you will receive the following benefits without any deductibles or copays. With this plan the patient or family will get:

• 2 exams: including comprehensive and periodic exams and necessary x-rays

• 2 emergency exams: including necessary x-rays

• 2 basic cleanings

• 2 oral cancer screenings

• 1 full mouth series of x-rays or panorex

All included in your annual membership. And additional discounts on the plan will be

15% off our UCR (Usual, Customary, and Reasonable) fees. These services include:

• Additional Cleanings / Periodontics

• Dental Sealants

• Fillings / Bonding

• Crowns

• Root Canals

• Extractions

• Cosmetic Veneers

• Dentures

• $1000 off - Invisalign® or Braces

• $100 off - Power Whitening

The annual membership fee is $299 per year for an individual.

 

For a married couple, it will be $499 per year and any dependents under the age of

24 years old will be $229 each.

Please be aware that this annual membership fee must be paid in full at the time of enrollment and all fees for treatment will be due and payable at the time of service.

This plan is effective for one year and is non-refundable.

One month before expiration, our members will be notified that their membership is expiring and can be renewed at that time.
